The Role
As a Physical Therapy Assistant in our home health program, you’ll work under the direction of a licensed Physical Therapist to deliver hands-on, compassionate care to patients in their homes. You’ll help patients recover mobility, manage pain, and achieve their rehabilitation goals, making a tangible difference in their daily lives.
Key Responsibilities:
Implement individualized treatment plans developed by Physical Therapists, including therapeutic exercises, mobility training, and pain management techniques.
Assist patients in their homes with activities to improve strength, balance, and functional independence.
Monitor and document patient progress, communicating updates to the supervising Physical Therapist and interdisciplinary team.
Educate patients and families on safe home exercise programs and adaptive techniques to support recovery.
Maintain accurate, timely documentation in compliance with home health regulations and standards.
Advocate for patients’ needs, ensuring they have the resources and support to achieve their goals.
Who We’re Looking For
You’re a skilled PTA who loves helping patients overcome physical challenges but feels stifled by rigid clinic schedules or impersonal care settings. You want a role where you can connect with patients in their homes, with the autonomy to make a difference and the support to grow.
Qualifications:
Active Ohio PTA License in good standing.
Graduate of an accredited Physical Therapy Assistant program.
Minimum of 1 year of PTA experience (home health or outpatient rehab experience preferred).
Strong knowledge of therapeutic techniques, mobility aids, and rehabilitation principles.
Excellent communication and interpersonal skills to build rapport with patients and families in diverse home settings.
Ability to work independently with strong organizational and problem-solving skills.
Valid driver’s license and reliable transportation (travel within Southeast Cuyahoga County, Hudson, and Aurora required).
CPR certification required.
